Focal epithelial hyperplasia/Heck disease. Acanthotic squamous epithelium with widened, confluent rete ridges, mild koilocytic change, and mitosoid bodies (degenerating nuclei mimicking mitoses). Usually caused by HPV types 13 and 32.
