Trichodysplasia spinulosa: follicles plugged w/ hyperkeratotic & parakeratotic debris, excessive inner root sheath differentiation towards keratinization. Enlarged cytoplasmic trichohyalin granules. EM shows intranuclear icosahedral polyomavirus inclusions!
