Myocardial Infarction Mechanical and Electrical Complications

Mechanical Complications - Early Complications (Hours – Days):

 • Cardiogenic Shock

 • Myocardial Free Wall Rupture (Pseudoaneurysm: LV defect contained by only pericardium/scar, more prone to rupture than aneurysm)

 • Interventricular Septal Rupture -> VSD

 • Papillary Muscle Rupture -> acute MR

Mechanical Complications - Late Complications (Weeks-Months):

 • LV Aneurysm (can be acute or chronic)

 • LV Thrombus

 • Pericarditis

Electrical Complications:

 • Bradyarrhythmias: Sinus bradycardia, First degree AV block, Second degree AV block: Mobitz Type I + II, Third degree AV block

 • Intraventricular Conduction Blocks

 • Supraventricular Arrhythmias: Sinus tachycardia, Atrial premature beats, Atrial fibrillation

 • Ventricular Tachyarrhythmias:  Premature Ventricular Contractions, Accelerated Idioventricular Rhythm (AIVR), Ventricular Tachycardia, Ventricular Fibrillation
