Indications for kidney transplantectomy / allograft nephrectomy:

Before 12 months from transplantation: 

Early graft loss, Vascular thrombosis, Severe acute rejection, Hyperacute rejection, Recurrent urinary infections or sepsis

After 12 months from transplantation: 

Signs of chronic infiammation, Graft loss due to BK virus nephropathy and a high level of BK viraemia, Graft intolerance syndrome, Cancer, Recurrent urinary infections or sepsis
