Hemophagocytic Lymphohistiocytosis (HLH)
Clinical Features: Fever, Splenomegaly, Hepatomegaly, Lymphadenopathy, Confusion
Classic lab findings in HLH: ↑↑ ferritin, Anemia, Thrombocytopenia, Neutropenia, ↑ AST & ALT, ↑ LDH, ↓ Fibrinogen & ↑ d-dimer, ↓ NK cell activity, ↑ soluble IL-2 receptor
Diagnostic Criteria for HLH (The Histiocyte Society Criteria):
Molecular diagnosis consistent with HLH OR
Meeting ≥ 5 of the following 8 criteria:
 • Fever
 • Splenomegaly
 • Cytopenias (affecting 2 2 cell lines)
 • ↑ ferritin
 • ↑ triglycerides and/or ↓ fibrinogen
 • ↓ NK activity
 • ↑ soluble IL-2 receptor
 • Hemophagocytosis on biopsy
