Hemophagocytic Lymphohistiocytosis (HLH) Treatment
Primary HLH:
- HLH-94 Protocol
• Dexamethasone
• Etoposide
• Cyclosporine (sometimes omitted)
• +/- intrathecal methotrexate (depending on CNS involvement)
- Followed by hematopoietic cell transplantation
Secondary HLH:
• Approach varies depending on trigger
• Almost all patients receive steroids
• In mild cases associated with infection or autoimmune disease, treatment of underlying condition may spare patient more toxic therapy
• Options for severe cases include: HLH-94, anakinra, IVIg, rituximab, alemtuzumab, tacrolimus
• Hematopoietic cell transplantation for refractory disease
