A clinical mnemonic to estimate the risk of major bleeding with anticoagulation in atrial fibrillation. A higher total score indicates a greater risk of major bleeding and argues against anticoagulation.
H - Hypertension (uncontrolled)
A - Abnormal renal or liver function (1 point each)
S - Stroke
B - Bleeding (prior major bleed)
L - Labile INRs
E - Elderly (Age > 65)
D - Drugs (Medications predisposing to bleeding, NSAIDs, anti-platelets) or Alcohol (1 point each)
