 - Waldenstrom macroglobulinemia (most common) / Plasmapheresis 
 - Multiple myeloma / Plasmapheresis 
 - Leukemia with blast transformation (CML) / Induction chemo or if unable, Leukapheresis + Hydroxyurea 
 - Leukemia with hyperleukocytosis (AML) / Induction chemo or if unable, Leukapheresis + Hydroxyurea 
 - Thrombocytosis / Plateletpheresis 
 - Polycythemia Vera / Phlebotomy 
 - MH/CCF/CWRU EM Res
