Step 1: Consider whether Posm may actually be normal or elevated

Step 2: Check Uosm  and/or Urine Specific Gravity

Step 3: Assess volume status and measure UNa

Step 4: Consider additional tests

Step 5: If SIADH is considered likely, the cause must also be determined
