Castleman's Disease and Associated Syndromes

HYALINE VASCULAR VARIANT

 • unicentric, HHV8-ve, children and young.

 • Hyalinized vessels

 • Expanded mantle zone in concentric fashion- onion skin

 • Atrophic erminal center

 • Hyalinizeä blood vessel penetrating the follicle together-lollipop

 • Dysplastic FDC

 • Systemic symptoms usually absent

 • Pathogenesis: FDC abnormalities, Increased VEGF

 • 10-20% of FDC sarcoma are associated with Castleman's disease

PLASMA CELL VARIANT

 • Hyperplastic germinal center

 • Diffuse plasma cells between follicles

 • Plasmablastic subtype is HHV8 +

 • Plasma cell variant can have hyaline vascular change- Mixed variant

 • Multicentric>unicentric

 • Systemic symptoms usually present

 • Pathogenesis

   "IL6 syndrome"

   Plasma cell disorders (myeloma, POEMS)

   HHV8, "Plasmablastic variant"

   Autoimmune diseases
