Upper Gastronintestinal Bleed - Differential Diagnosis Algorithm

Acute Hematemesis/Melena

 - Blood in vomitus?/black, tarry stools

 - If Melena, 5-10% colorectal/small bowel.

 - Exclude bleeding disorder.



Peptic Ulcer Disease (55%):

 - Gastric Acid Hypersecretion

    • Zollinger-Ellison

 - Non-Steroidal Anti-Inflammatory Drugs

 - Stress (ICU Setting)

 - Helicobacter Pylori

Portal Hypertension (15%):

 • Gastro-esophageal varices

Other:

 • Mallory Weiss Tear

 • Esophagitis / Gastritis

 • Tumors
