Biliary Duct Obstruction

 • Autoimmune— PBC, PSC

 • Infection— liver fluke, HIV, CMV, ascariasis

 • Malignancy— cholangiocarcinoma

 • Other— choledocho/ithiasis

Intestinal Wall Infiltration

 • Malignancy— lymphoma

 • Autoimmune— Crohn's disease, celiac disease

 • Immunodeficiency— CVID

 • Protein- amyloidosis

Other:

 • Cholestatic drugs- e.g. azathioprine, phenytoin



Pancreatic Exocrine Insufficiency - Acquired

 • Chronic pancreatitis

 • Pancreatic resection

Pancreatic Exocrine Insufficiency - Hereditary

 • Hemochromatosis

 • Shwachman-Diamond syndrome

 • Cystic fibrosis

 • Pancreatic lipase deficiency

Pancreatic Exocrine Insufficiency - Malignancy

 • Pancreatic adenocarcinoma

 • Gastrinoma

Autoimmune

 • Celiac disease

 • IBD- CD, UC

Altered length

 • Short bowel syndrome

 • Small bowel resection

Infectious

 • SIBO

 • Giardiasis

 • Whipple' disease
