Causes of Metabolic Alkalosis - Differential Diagnosis Algorithm

Urine chloride < 20 mmol/l - Chloride responsive (Chloride depletion):

 • Vomiting

 • NG suction

 • Chloridorrhea

 • Cystic fibrosis

 • Remote diuretic use

Urine chloride > 20 mmol/l - Chloride resistant:

 • Current diuretic use, Severe K depletion, Exogenous alkali, Mg deficiency

 • Increased BP = Hyperaldosteronism

 • Decreased BP = Bartter's Syndrome, Gitelman's Syndrome
