Approach to the Hypercoagulable State

Arterial thromboembolism

→ Cardiac rhythm monitoring, Echocardiogram, Vascular imaging

→ Test for APS, particularly in younger patients and in those with a history of prior venous thromboembolism (VTE).

(Inherited thrombophilias are not a major risk actor for arterial thrombosis)

Venous thromboembolism:

 • Provoked (e.g. surgery, trauma, active malignancy, immobilization)

    - Age-appropriate cancer screening (in absence of an established malignancy)

    - No other workup typically indicated

 • Unprovoked

    - Everyone → Age-appropriate cancer screening, CBC, aPTT, +/- CXR

    - Recurrent VTE → Inherited and APS

    - Age < 45-50 years of age → Inherited and APS
