Janeway Lesions in Endocarditis seen on Physical Exam

36 year old male with history of illicit drug use. Presented with sudden left hemiplegia, fevers.  Blood cultures growing MRSA.

His sister shared these photos today taken 15 days prior to presentation - Look at the characteristic Janeway lesions. 

Infective Endocarditis could have been diagnosed much earlier in an IV drug user with persistent fever - Clinical examination is so so important.
